- Copy of Company PAN Card
- Copy of latest Company Address Proof (Utility Bill, Bank Statement or Lease Agreement — not older than 2 months)
- Board Resolution (BR) on company letterhead in the prescribed format
- Beneficial Ownership Declaration on company letterhead (any shareholder with more than 10% ownership must be included)
- List of Directors on company letterhead
